Online application form for UP State Health Card scheme or Cashless treatment scheme for UP state govt. employees and pensioners are being accepted through the official website of State employees cashless treatment scheme (SECTS) at upsects.in or uphealth.up.nic.in.
State employees cashless treatment scheme launched in the name of Pandit Deen Dayal Upadhyay. The scheme is also known as Cashless Suvidha. Interested employees and pensioners can apply online for cashless treatment facility by following the registration procedure.
Under the UP Health Card Scheme, the government will provide medical emergency like accident, angioplasty, valve replacement, joint replacements, Organ transplant types treatment etc. And there will be no money limit of the treatment. And the facility will be available all over India for UP government employees.
Step-1: Visit the official website at upsects.in and go to "Employees / Pensioner Gateway" and click on "Apply for State Health Card" or use the direct link to apply online for UP health card.
Step-2: You will see the online application form of UP state health card scheme as available in the image below. In this form, you will have to enter all the mandatory details of yours like DDO Code, office name, applicants name, DOB, Basic pay, Aadhar number etc.
Step-3: After that click on check box and enter the verification code. and then push the submit button.
Now your online form for UP health card is completed but to get the heath card, you will have to visit the (Withdrawal Officer / Treasurer) for the verification of the application. And after registration verification, the beneficiaries would be able take a print health card from www.upsects.in.

After verification, the beneficiary can print UP health card by using the link below. To you print the health card, you will have to enter your Aadhar Number
after that an OTP (One Time Password) will be sent to you. After
receiving it, enter the same code in the required field and pint your
cashless treatment card.
